The legislation further requires the camp to maintain a record of the parental permission and allows camp staff to assist with the application of sunscreen/bug spray when the child is unable to do so, provided the child requests the assistance and that this assistance is permitted/authorized by the parent.                                                                                                     REQUIRED DOCUMENTATION By law, campers may not be at camp without the  following:• Current Physical Examination & Immunization History• Health History & Insurance Information (online form)• Medication Administration & Over-the-Counter Medication Form (*must be signed by both parent and health care provider*)All required medical information must be sent to the Health Director. Included should be your child's most recent physical, up to date immunization record, any medical notes (i.e. if your child has a medical condition), and any orders for medication distribution (only if your child needs to be distributed medication while at camp, must include medication, child's name, doctor's name, dosage, and time to be administered.
